Authorization and Release Form

Please read the following Authorization and Release Form Terms and Conditions before electronically signing and submitting the form.

1. I understand my testimonial, endorsement images, and other media, may be used in connection with publicizing and promoting Summit Systems.

2. I authorize the use of my name, photo, and likeness, together with my endorsement or testimonial, if provided, whether written or oral, which may appear in any media released by Summit Systems in connection with any advertisement or promotion of Summit Systems including, but not limited to, brochures, website, and social media content.

3. I understand that my name will be disclosed in conjunction with my testimonial, endorsement, or other media.

4. I agree that Summit Systems may copyright my endorsement, testimonial, or other media in its name.

5. I authorize Summit Systems to use, reproduce, exhibit, and distribute any print or digital promotional materials containing my name and photo, together with my endorsement or testimonial, for the purpose of endorsing its services.

6. I hereby certify that my endorsement, testimonial, or other media provided of Summit Systems’ services is true and accurate and reflects my personal experience and honest belief in the quality of the service that I am endorsing, providing a testimonial, or other media. I am doing this testimonial or providing media of my own free will and I have not been paid a monetary sum for doing so.

7. I hereby certify that I am over twenty-one years of age and competent to contract in my own name.

8. I understand that this Authorization and Release Form may not be amended without my written consent and the written consent of Summit Systems.

9. I have read and fully understand this Authorization and Release Form and I am signing it voluntarily.
